Hartford Foundation Awards Grants Totaling $6 Million to Improve Geriatric Care
The John A. Hartford Foundation has announced three grants totaling more than $6 million to nonprofits working to make changes in healthcare practice that improve the health of older adults.
The grants include an award of more than $5 million to the Gerontological Society of America to create the Hartford Change AGEnts Initiative, an effort to engage, connect, and inspire at least five hundred Hartford Foundation grantees to lead changes in healthcare delivery that improve the health of older adults; and an award of $584,150 over two years to Community Catalyst, Inc. to support the engagement of geriatrics experts in its Voices for Better Health project, which advocates for evidence-based, high-quality health plans for dually eligible beneficiaries of Medicare and Medicaid. With the grant, which augments a $3 million award from Atlantic Philanthropies, Community Catalyst will select and fund at least ten leaders in the geriatric healthcare field in five states to serve as advocates for integrated care plans that improve the lives of vulnerable older adults.
The foundation also granted $415,422 over eighteen months to the National Committee for Quality Assurance in support of the first stage of a multi-phase initiative that aims to develop outcomes-based performance measures designed to improve the quality and experience of care for dually enrolled Medicare/Medicaid beneficiaries.
