The Right Place: An Overview of Supportive Housing Options for Seniors and People with Disabilities

Supportive housing options can play an important role in the coordination of healthcare delivery for seniors and people with disabilities, a report from the California HealthCare Foundation argues. Released in advance of the 2014 launch of California's MediConnect program — a joint effort of Medi-Cal and Medicare to provide dual-eligible beneficiaries with medical, health, long-term institutional, and home- and community-based services through a single system — The Right Place: An Overview of Supportive Housing Options for Seniors and People With Disabilities (38 pages, PDF) offers an overview of several options, including licensed residential care facilities, as well as unlicensed options such as income-restricted apartments, permanent supportive housing, and the village model. The report examines how managed care plans in other states use supportive housing to optimize services and makes recommendations for plans participating in MediConnect, including developing a case management system, focusing on residential care facilities with the capacity to retain high-need residents, and working with care transition agency staff who know the local affordable housing market.
